Session Structure Agreement:
I appreciate the recommendation for weekly sessions to help build a consistent and safe space for growth. I understand that this is flexible and can be adapted to suit my needs.
Fee Information:
I am informed about the session fee of $200 for 45 minutes and $240 for 60 minutes, and understand that this is an out-of-network service. I acknowledge that a monthly superbill will be provided to assist with potential insurance reimbursement.
Financial Considerations:
I understand that sliding scale options may be available but are limited. If I have financial concerns, I will mention them during our initial consultation for further discussion.
